Step-by-step Instructions

New Legal Form

  1. Log into Registration.
  2. Select the School Info tab from the left-side navigation menu.
  3. Select the 🔹blue New Legal Form to add a new legal form.
  4. Enter the name of your legal form in the Name box.
  5. Enter the specific checkbox verbiage you want your parents to consent to in the Checkbox Text box for example, I consent to the concussion form.
  6. Select if you want students to sign off and if you want participants to receive an email copy of the legal form.
  7. Select the grey Choose File to upload your legal form.
  8. Click the 🔹blue Create Legal Form button.
  9. Your legal form will now be available to select from when you begin setting up your activities and sports for registration.

New Medical Form

  1. Log into Registration.
  2. Select the School Info tab from the left-side navigation menu.
  3. Select the 🔹blue New Medical Form to add a new medical form.
  4. Enter the name of your medical form in the Name box.
  5. Select the grey Choose File to upload your medical form.
  6. Click the 🔹blue Create Medical Form button.
  7. Your medical form will now be available to select from when you begin setting up your activities and sports for registration.

    NOTE: Admins are not able to archive Medical forms. If you need a form archived/removed - please reach out to Snap! Manage support, and we'd be happy to assist!

Custom Verbiage for Medical Forms - optional

When parents/guardians register and get to the medical form step they will see the system's default verbiage. You have the option to change this and add your own verbiage. Some schools will add specific instructions for parents to follow after the physical has been downloaded. Follow the steps below to customize your verbiage. 

  1. Log into Registration.
  2. Select the School Info tab from the left-side navigation menu.
  3. Select the 🔹blue Customize Verbiage.
  4. Type your verbiage in the Verbiage box.
  5. Click the 🔹blue Update Medical Form Verbiage.

New Legal Video

All videos must be a YouTube file. Videos will be shown for all open/registerable activities. 

  1. Log into Registration.
  2. Select the School Info tab from the left-side navigation menu.
  3. Select the 🔹blue Add New Legal Video button.
  4. Add the video title in the Video Title box.
  5. Enter the video display text in the Video Display Text box Note: This will appear below the video.
  6. Copy and paste the Youtube share link into the Youtube Share Link box.
  7. Check the require student sign off box if you want students to consent to the legal video.
  8. Select the 🔹blue Create Legal Video button. 

    NOTE
    : Admins are not able to archive Legal Videos. If you need a form archived/removed - please reach out to Snap! Manage support, and we'd be happy to assist!

FAQ's about medical and legal forms

My PDF upload won't upload or save; what do I do?

All titles must be original when uploading legal and medical forms. If you have an archived legal form that shares the same title as the new legal form you are adding, then your legal form will not be saved. You will need to adjust the title so it is different from the archived legal form.

I don't see an option to archive/remove my legal video or medical forms; what do I do?

Due to the nature of these file types, admins cannot archive these files in Registration. If you would like a form removed, please reach out to support; we'd be happy to help!

Can I select which activities to view the legal video during registration setup?

This is not a feature our platform supports at this time! Legal videos will be shown to all registration users, regardless of activity or activity type.
