Uploading a Physical Form: Scanning or Combining Photos into One PDF

When uploading your child’s physical form, our system accepts a single PDF file.
If your form is multiple pages, you can either scan it or take photos of each page and combine them into one PDF—right from your phone. Most smartphones have built-in tools to do this (no paid app needed).

Option 1: Use Your Phone’s Built-In Scanner

On iPhone (iOS 15 or later)

  1. Open the Notes app.

  2. Tap the New Note icon.

  3. Tap the Camera icon → Scan Documents.

  4. Position the form page in view; it will auto-scan.

  5. Repeat for each page.

  6. Tap Save → your scans are saved as one PDF in the note.

  7. Tap Share → Save to Files or Email the PDF to yourself.

On Android (Google Drive app)

  1. Open the Google Drive app.

  2. Tap the + (New) button.

  3. Select Scan.

  4. Position the page and tap the shutter button.

  5. Tap + to add more pages.

  6. When done, tap Save → choose a folder in Drive.

  7. Open Drive, select the file, and download it as a PDF.

Option 2: Combine Photos into One PDF (if already taken)

On iPhone

  1. Open the Photos app and find the images you took of the physical form.

  2. For each image, tap it → Share icon → scroll down → Save to Files.

    • Make sure all your form photos are saved in the Files app before moving to the next step.

  3. Open the Files app.

  4. Locate and select all the saved photos.

  5. Tap the More (…) menu → Create PDF.

  6. Your photos are now one PDF, saved in Files.

On Android

  1. Open the Google Photos or Files by Google app.

  2. Select all the images.

  3. Tap the Share icon → Print.

  4. From the Printer menu, choose Save as PDF.

  5. Save the file to your device or Google Drive.
